I’m guessing an OOB det. Possibly a broken casing lodged in the chamber. Just one of the reasons I don’t like the forward assist button, lets a guy like that just hammer a round into the chamber far enough for the firing pin to do it’s thing, but not get the round fully chambered.
Can’t tell from here, but I wonder if he didn’t pop a round off the crimp and into the case when he was mashing the forward assist like a meth addled monkey.
That could cause an overpressure failure.
That forward assist is just to help a grungy bolt or a cruddy round into the chamber. If you need do to a kung-fu palm strike to the forward assist there’s probably more wrong with your gun than brute force can solve!
I agree with Weer’d. If you think you need to pick up a rock to work your forward assist, you should probably stop and assess. I don’t even use the forward assist on mine. I act like it isn’t there.
