Back in 1990 or so, my wife was a resident at Johns Hopkins in Baltimore. A shooting happened in the ER waiting room there, where an armed hospital guard came upon his wife and a sick man, her boyfriend, she brought to the ER for treatment. Oopsie.
I don’t recall if all 3 ended up dead, but my wife still talks about how it took a whole hour of talking with police to get another resident to un-barricade the storeroom she had occupied and fortified when the shooting started.

Dude killed his ex-wife and her boyfriend. Good chance he would have done himself next, even without intervention.
